Requests for Medical and Billing Records:

We do not maintain patient medical records and are not the custodian of medical or physician billing records. Requests for medical or billing records submitted directly to SFEMA will not receive a response, as we are unable to process these requests.

Medical Records

All requests for medical records, including requests from attorneys, law firms, insurance companies, or other third parties, must be submitted directly to the hospital where the patient received care. SFEMA does not have access to hospital medical records and cannot fulfill these requests.

Physician Billing Records

Requests for physician billing records must be submitted electronically through ChartSwap, the HIPAA-compliant records request platform.

To request billing records:

Register as a Record Requestor at www.chartswap.com.

Sign in, search for the applicable provider, enter your request details, and upload any required supporting documentation.

Once your request has been reviewed and records are available, you will receive a notification and an invoice, which may be paid by check or credit card.

Please note that requests for billing records are not accepted by mail, fax, email, or telephone. All billing record requests must be submitted through ChartSwap.
